Parent/guardian's name and email address for invoicing. 2. Child's name, age and gender. 3. Adjectives that best describe their child's voice (eg: brave, confident, kind). IMPORTANT NOTES: -Billing details will be sent during the week of June 29th and payment is due upon receipt. No refunds or credits, etc. once booked. -Scripts and further details will be sent by melody@khcasting.com during the week of July 20th.
